Patient Sortal is a digital health provider that delivers primary care to formerly incarcerated adults as they return to the community after prison. Founded in 2018, the company provides virtual primary care, chronic care management, telehealth appointments, prescription refills, and medication management, along with referrals to vision, dental, behavioral health, substance abuse, and other specialist providers. Members can see a provider within 14 days of release, with or without insurance, and the company partners with over 30 Medicaid, Medicare, and commercial health plans across its current service area of Pennsylvania and New Jersey.
The company's mission is to bridge the gap between release from prison and access to Medicaid, easing reintegration and reducing the burden on emergency healthcare services. To do this it has built a Population Health Model of Care for justice-involved individuals, supported by Johns Hopkins Medicine and the Johns Hopkins ACG system. According to Eagle Venture Fund's profile, about 800 returning citizens receive healthcare through Patient Sortal, roughly 3,000 more are on its waiting list, and its state Medicaid partnership is projected to have saved over $2 million.
